With effect from the 1st April 2017 we will be calling every patient in for their review during their month of birth.  This means that although you may have had a review in February, because your birthday falls in July we will be calling you in again.  We are hoping that this will also help you remember when you are due for your annual checks and you will know when you should attend.  We will try to book all your reviews in together to avoid you attending surgery on multiple occasions.  Please remember that if you are aged 40-75 you may also be booked in for an annual health check at the same time.

For patients that do not currently have any Long Term Conditions, we will invite you in for screening. For example we will invite you in for screening, to identify if you are at risk of developing a long term condition, such as ' diabetes'.  It is very important that you engage with the practice, once the screening is completed, we will have a result, if the result is marked see ' Health Trainer' it is very important that you attend to see the Health Trainer or a member of the nursing support team, these trained members of staff will help you make lifestyle changes to help lower your risk of getting a long term condition such as diabetes.
